Grantee Reporting Forms and Instructions
Please note that all reports must be submitted via our new Grantee Portal.
To submit your report:
• Check the portal and see if you have an account: DSYF Grantee Portal
• If you don’t have an account complete the online Portal user registration form.
• Once submitted, you will receive an email invitation to complete the registration process and upload your report.
• Report forms are linked on the homepage of the portal.
If you already have a portal account but can’t remember your username, try using the prefix of your email address (before the “@”) as your username and reset your password using the portal’s “Forgot Password” feature. If you need further assistance, please email us at portal@stuartfoundation.org.
